To receive hospice care, a patient must have:

Diagnosis of terminal illness (a
disease with no cure)

Referred by attending physician

Consent of patient and/or primary
caregiver

Residence within the service area
-
Limited life expectancy
Hospice referrals
may be initiated by patients, family members, friends or health care
professionals. |
