 To receive hospice care, a patient must have:

• Diagnosis of terminal illness (a disease with no cure)

• Referred by attending physician

• Consent of patient and/or primary caregiver

• Residence within the service area

• Limited life expectancy

     Hospice referrals may be initiated by patients, family members, friends or health care professionals.
