which include assist with bathing, grooming, erxercise and
meal preperation.
- Social Services that focus on social &
emotional factors related to health condition. Assist with
long range planning and community resourses available and
working with family members

To receive Home
Health, a patient must have:
- A skilled nursing or physical therapy
need.
- Physician referral.
- Homebound: Dependant on type of payor.
Definition of homebound is a patient who has difficulty
leaving their home, that it is difficult and tasking and
only leaves for short periods of time.
Sakakawea Home Health provides free in home
evaluations to determine criteria.
Home Health referrals may be initiated by physician, patient,
family member or health care professionals.
